NVOG richtlijn - Geneeskunde - Pijnbehandeling bij de bevalling
Als er hoge pijnscores tijdens de bevalling worden aangegeven is er beschikbaarheid van adequate pijnbehandeling tijdens de bevalling mogelijk. Tijdens de zwangerschap dient iedere vrouw geïnformeerd te worden, over de mogelijkheden van pijnbehandeling tijdens de baring. Pijnscores worden aangeduid als baringspijn, waarbij er hevige pijn in de buik optreedt ter gevolge van weeën. Deze weeën nemen meestal toe tijdens de ontsluitingsfase. De pijnintensiteit wordt beïnvloed door het stadium, duur en gebroken vliezen. Ook het gewicht van moeder en kind, menstruatiepijn voor de zwangerschap en sociaal-demografische kenmerken kunnen ertoe leiden dat iemand meer pijn ervaart. Tevens de zorgverlener; de attitude naar de barende vrouw en de omgeving spelen een rol in de pijnintensiteit.
Het kunstmatig breken van de vliezen gaat gepaard met minder ondraaglijke pijn dan bij vrouwen waarbij dit niet werd uitgevoerd.
Adequate pijnbehandeling bestaat uit goede emotionele ondersteuning. Dit wordt gegeven in combinatie met effectieve pijnstilling. Pijnbehandeling van de eerste keuze bestaat uit epidurale analgesie. Epidurale analgesie heeft een goede balans qua effectiviteit van de pijnbehandeling en de veiligheid voor de moeder en kind. Het is belangrijk dat de vrouw geïnformeerd wordt over de kans dat epidurale analgesie een grotere kans geeft op het gebruik van oxytocine. Daarnaast kan epidurale analgesie leiden tot een langere uitdrijvingsduur, een verhoogde kans op een vaginale kunstverlossing, verhoogde kans op een lage bloeddruk en urineretentie. Daarnaast gaat epidurale analgesie gepaard met een verhoogde kans op een stijging van de temperatuur. Als de vrouw een verhoogde temperatuur heeft, kan een antibiotica behandeling nodig zijn.
Het is belangrijk dat pijnbehandeling tijdens de baring adequaat als tijdig is. in overleg met de vrouw wordt besproken welke vorm van pijnbehandeling adequaat is.
