Achtergrond
Chronische PTSS is een veel voorkomende aandoening die mensen kunnen ontwikkelen na uitzonderlijk bedreigende en verontrustende gebeurtenissen. De relatieve effectiviteit van de verschillende psychologische behandelingen voor chronische PTSS is onduidelijk. In dit artikel worden een review en meta – analyse gepresenteerd van randomised control trials die de effectiviteit van psychologische behandelingen onderzoeken in het verminderen van symptomen van PTSS. Ook wordt de effectiviteit van verschillende psychologische behandelingen in het verminderen van PTSS vergeleken.
Methode
In een systematische review van randomised control trials, werden de studies die in aanmerking kwamen beoordeeld op basis van methodologische kwaliteitscriteria. De data werden hierna geëxtraheerd en geanalyseerd.
Resultaten
In de meta – analyse werden 38 studies opgenomen. Trauma – focused cognitieve gedragstherapie (TFCBT), Eye Movement Desentisation and Reprocessing (EMDR), stress management en cognitieve - gedragstherapie in een groep verbeterden de PTSS symptomen meer dan wachtlijsten of de controlegroep. Er was geen overtuigend bewijs voor andere therapieën. Er was geen bewijs voor verschil in effectiviteit van TFCBT en EMDR maar er was wel wat bewijs dat TFCBT en EMDR superieur zijn aan stress management en andere therapieën, en dat stress management superieur was aan andere therapieën.
Discussie
De eerste lijn psychologische behandelingen voor PTSS zouden moeten zijn gericht op trauma (trauma – focused). TFCBT of EMDR zijn voorbeelden hiervan.
